Congenital Esophageal Atresia with Tracheoesophageal Fistula


Abstract
The congenital esophageal atresia with tracheoesophageal fistula was described first by Durstone in 1670 and has been collected of about 2, 000 cases in the world literatures upto date. However, the result of treatment is not completely satisfactory until now. There are 30 reported cases of congenital esophageal atresia with tracheoesophageal fistula in Korea and the first survival case in Korea was reported by us in 1970. We have experienced 3 cases of. esophageal atresia with tracheoesophageal fistula during recent years who are survived the surgical correction and in good condition as far as the esophageal situations are concerned.

All three cases were normal in weight at birth and two of them were associated with cardiac anomalies, one with dextrocardia and the other with undetermined congenital heart disease. The type of deformities of esophagus was classified as Gross¢¥s type "C" in all 3 cases. The common predominent symptoms were nausea, vomiting, cough and cyanosis. The diagnosis was confirmed by the esophagogram in all 3 cases. The corrective surgery was performed within 30-120 hrs. after birth. The surgical technique followed the methods of Haight¢¥s telescopic end-to-end anastomosis. The simultaneous feeding gastrostomy was performed in two cases. The immediate postoperative course were uneventful in all three cases. There is mild stenosis in one case which is tolerable by the patient. The last case showed moderate degree of stenosis at the anastomosis site since about three weeks after the operation which was dilated with Tucker dilator three times already in three weeks interval.

The literature review is made and some discussions about the particular problems of this not unfrequent disease is done.
